Our Privacy and Confidentiality Agreement

Groff & Associates, LLC  provides wellness solutions for its clients.  As such, we uphold the strict guidelines and policies for our providers and clients in order to maintain Federal and State guidelines.  We believe that privacy is client information that is NOT available for public use and the right of individuals to protect sensitive information about themselves.  We expect our providers, partners, contractors or any other relationship to adhere to our standard policies and procedures on Privacy and Confidentiality. 

Confidentiality is expected of our providers as they enter into a contractual agreement with a client.  We believe that confidentiality is the responsibility of our providers and our organization.  The exceptions to confidentiality are:

  • Client consents to a release of information.
  • Provider suspects child or elder abuse.
  • Provider has an obligation to warn any individual who may have intent to harm themselves or others.
  • Disclosure is made to medical personnel in a medical emergency or court order.

This agreement may change depending upon Federal and State regulatory changes or our standard policies and procedures.  Please review our policy on occasion to insure you are aware of any changes.
