Physical Health Provider Professional Standards

Groff & Associates  provides Employee Assistance Program Wellness Solutions.  We desire to provide the best possible healthcare for our clients and require all providers meet state licensure requirements for the state they provide services within. 

PHYSICAL HEALTH PROFESSIONAL QUALIFICATIONS

1. All Chiropractic providers will have a minimum of a master's degree or having graduated from an accredited program of study. 

2.  All Massage Therapy providers must be 18 years or older.  Must have a high school education or equivalent.  Must have successfully completed a massage therapy school program that requires a minimum of five hundred (500) hours of supervised classroom and hands-on instruction of massage therapy.

3.  All Fitness Trainers must be 18 years or older.  Must have a high school education or equivalent.  Must have at least two (2) years of practical hands-on experience in weight resistance and/or cardio training.

4. All Chiropractic, Massage Therapy and Dietitian providers must be licensed/certified by the state in which they practice.

5. All Chiropractic and Massage Therapy providers will furnish proof of coverage for liability insurance.

The minimum levels of coverage are as follows:

Agencies/Organizations: Where coverage is in the name of an organization or agency and the individual providers are covered as employees of that agency, the minimum level of liability coverage will be $1 million per occurrence (claim)/ $3 million per aggregate, or will comply with state laws and regulations.

Individuals: Individuals will carry a minimum of $1 million per occurrence (claim)/$1 million aggregate.

Physicians: Chiropractic physicians will carry a minimum of $1 million per occurrence (claim)/ $3 million aggregate.

Providers must provide supporting documentation to the above minimum qualifications.
